Before participating in a retreat with Adytum Retreats LLC, individuals should consult with a physician. Individuals under eighteen years of age must be accompanied by a parent or legal guardian who has signed this form on behalf of the minor.

Voluntary Participation

By signing below, you understand, and acknowledge that you have voluntarily chosen and requested to participate in a retreat, class, workshop, or activity sponsored by Adytum Retreats (the “Event”).

Acknowledgment and Assumption of Liability

I acknowledge and understand there are risks inherent in participating in the Event, including and without limitation the risk of sustaining injury while participating in the Event. I represent that I possess the level of health, strength, and flexibility necessary to participate in the Event, and I understand and acknowledge there still remains a risk of injury. I agree to assume and accept all risks and further agree to assume all responsibility and liability for any and all injuries I may sustain due to my participation in the Event.

I further acknowledge and understand there are inherent risks associated with travel and group gatherings, including and without limitation the risk of contracting Covid 19 or any other infectious disease. I assume all responsibility and liability for any and all damages and costs that may accrue while participating or related to my participation in the Event.

Release, Waiver, and Indemnity

In consideration for being permitted to participate in the Event, I and my legal representative(s), successors, and/or assigns, agree to waive, release, and hold harmless Adytum Retreats LLC, its principals, officers, governors, managers, members, agents, instructors, employees, and other Event participants, from any liability, loss, claims, and demands that may accrue from any loss, damage, or injury to my person or property, in any way resulting from, or arising in connection with, the Event.

Adytum Retreats has the right to print, reproduce and use images from retreats for personal and business purposes. These images may be distributed in print or on the internet in any form.

I have carefully read this Liability Release, Waiver, and Indemnity Agreement, fully understand its terms and contents, and understand I have given up substantial rights by signing. I have signed this release freely and voluntarily. I am at least 18 years of age or my parent or legal guardian is signing on my behalf.
